Form 36, Notice of Intention to Reduce or Discontinue Payments

Employers must submit Form 36 to notify the Connecticut Workers' Compensation Commission of their intent to reduce, discontinue, or terminate an employee’s workers' compensation benefits. A production company must file this form if adjusting payments after a crew member's recovery or return to work.
