Tax Status Report Application
This form is used by a corporation or its authorized representative to apply for a report from the NYC Department of Finance concerning the entity's tax status. All sections must be completed in their entirety for the application to be accepted. A Power of Attorney form must be submitted along with the application if an authorized representative is requesting the report.
