Connecticut Digital Media & Motion Picture Tax Credit Production Tax Credit Voucher Application

After completing a production, companies apply for a tax credit voucher by submitting this form within 90 days of incurring the last qualifying expense. The voucher can then be used to offset tax liabilities or be transferred.

Illinois Film Tax Credit Tracking Sheet

In order to qualify for Illinois film tax credits, applicants must submit a diversity plan detailing the manner in which the production proposes to achieve its goals to ensure employment of minorities that represent the State of Illinois rather than merely to assure nondiscrimination. Applicants must also document “good faith efforts” in attempting to achieve a racially diverse crew.
