This is a sample of Form FP, which can be used to apply for the Massachusetts film production incentive program. This printable version of Form FP should serve only as a sample. As of June 28, 2024, all Film Tax Credit applications should be submitted on MassTaxConnect, the Department of Revenue’s web-based application for filing and paying taxes.

Illinois Film Tax Credit Tracking Sheet

In order to qualify for Illinois film tax credits, applicants must submit a diversity plan detailing the manner in which the production proposes to achieve its goals to ensure employment of minorities that represent the State of Illinois rather than merely to assure nondiscrimination. Applicants must also document “good faith efforts” in attempting to achieve a racially diverse crew. Applicants must track progress in reaching the State’s diversity goals by using this "Diversity Plan Tracking Sheet.”
